Preparing for your first office visit is easy.
For your convenience, you can fill out our Intake Form before you come in and save time in the waiting room.
If you are a prior patient of Dr. Loranger, or if you have records from a previous eye doctor, please fill out our Records Release Form and send it to the previous office so we receive your medical history.
